Document number and revision clearly stated Date of issue and review date specified Author name and qualifications stated Approved by (competent person) signed Project name and location correct Activity description specific and accurate

Risk Assessment Quality (6 items)

All reasonably foreseeable hazards identified Risk ratings before controls are realistic (not artificially inflated) Control measures are specific and practical (not generic platitudes) Residual risk ratings after controls are justified Risk assessment references current regulations (CDM, WAH, COSHH etc.) Assessment is proportionate to the complexity of the activity

Method Statement Completeness (6 items)

Sequence of work is logical and complete Equipment and materials are specified Competency requirements are listed (CSCS, CPCS, trade cards) Supervision arrangements are described Duration and working hours are stated Dependencies on other trades are identified

Site-Specific Content (6 items)

References specific site conditions (ground, access, neighbours) Addresses site-specific hazards (buried services, contamination) Control measures are achievable with available resources Emergency procedures reference this specific site Weather considerations are addressed (if outdoor work) Not a copy-paste from a different project

Communication (5 items)

Written in language workers can understand Key points suitable for toolbox talk briefing Workers have signed to confirm they have read and understood Supervisors have copy and are briefed Document is accessible on site (not locked in office)
